If you ask health care providers about the most challenging condition to treat, chronic pain is mentioned frequently. By its nature, chronic pain is a complex and multidimensional experience. Pain perception is affected by our unique biology, our mood, our social environment, and past experiences. If you or a loved one is suffering from chronic pain, you already know the heavy burden. Given the ongoing challenges of chronic pain management coupled with the consequences of the opioid epidemic, pain management practitioners and their patients are searching for effective and safer alternatives to opioids to alleviate pain. With the legalization of marijuana in many states and resulting cultural acceptance of this drug for recreational and medical use, there has been an increased interest in using cannabis for a myriad of medical problems, including pain. Cannabis (most commonly obtained from the Cannabis indica and Cannabis sativa plants) has three major components: cannabinoids, terpenoids, and flavonoids. While there are over a hundred different cannabinoids, the two major components are tetrahydrocannabional (THC) and cannabidiol (CBD).

Historically more attention has been paid to the psychoactive (euphoric "getting high") component of the cannabis plant, THC; there have been fewer scientific studies on the medical use of CBD, a non-psychoactive component of the plant. What’s the thinking behind using cannabis for chronic pain? CBD is emerging as a promising pharmaceutical agent to treat pain, inflammation, seizures, and anxiety without the psychoactive effects of THC. Our understanding of the role of CBD in pain management continues to evolve, and evidence from animal studies has shown that [https://git.qiucl.cn/rosie80p737028 G.H. Farms CBD] exerts its pain-relieving effects through its various interactions and modulation of the endocannabinoid, inflammatory, and nociceptive (pain sensing) systems. The endocannabinoid system consists of cannabinoid receptors that interact with our own naturally occurring cannabinoids. This system is involved in regulating many functions in the body, including metabolism and appetite, mood and anxiety, and pain perception. What’s the research that CBD works in humans?

Given its promising results in animal models, along with its relative safety, non-psychoactive properties, and low potential for abuse, CBD is an attractive candidate to relieve pain. Unfortunately, there is a lack of human studies about the effectiveness of CBD. However, there is an abundance of commercial advertisements about the magical effects of CBD, and it is frequently presented as a cure-it-all potion that will treat everything including diabetes, depression, cancer, chronic pain, and even your dog’s anxiety! So far, pharmaceutical CBD is only approved by the FDA as adjunct therapy for the treatment of a special and rare form of epilepsy. Currently, CBD alone is not approved for treatment of pain in the United States. But a [https://www.academia.edu/people/search?utf8=%E2%9C%93&q=combination%20medication combination medication] (that contains both THC and CBD in a 1:1 ratio) was approved by Health Canada for prescription for certain types of pain, specifically central neuropathic pain in multiple sclerosis, and the treatment of cancer pain unresponsive to optimized opioid therapy. There is currently no high-quality research study that supports the use of CBD alone for the treatment of pain.

Why is CBD presented to the public this way, when it is not without risks? Given the rapid change in the legality of cannabis coupled with the increased appetite for something new, and driven by unprecedented profit margins, the advertising for cannabinoids in general and CBD in particular has gone wild. The FDA is very clear that it is illegal to market CBD by adding it to a food or labeling it as a dietary supplement. And it warns the public about its potential side effects, as it’s often advertised in a way that may lead people to mistakenly believe using CBD "can’t hurt." CBD can cause liver injury, and can affect the male reproductive system (as demonstrated in laboratory animal studies). Most importantly, CBD can interact with other important medications like blood thinners, heart medications, and immunosuppressants (medications given after organ transplantation), potentially changing the levels of these important medications in the blood and leading to catastrophic results, including death.

Also, more information needs to be gathered about its safety in special populations such as the elderly, children, those who are immunocompromised, and pregnant and breastfeeding women. In fact, the FDA has issued several warning letters to companies and individuals that market unapproved new drugs that allegedly contain CBD. The FDA has tested the chemical content of cannabinoid compounds in some of the products, and many were found to not contain the levels of CBD the manufacturers had claimed they contain. Finally, there is anecdotal wisdom, when experiences by patients and [http://dmnsj.com/bbs/board.php?bo_table=free&wr_id=370014 G.H. Farms CBD] health professionals have positive results. While the experience or medication could be beneficial, that doesn’t mean it is going to work for everyone. That’s because each and every person is unique, and what works [https://www.britannica.com/search?query=perfectly perfectly] for one patient could have no effect on another patient. This is especially true for pain, Global Health CBD Gummies where many other factors (our mood and stress level, our environment and Order Global Health CBD Gummies other medical conditions, and our previous experiences) can affect the perception of pain. If you or someone close to you is considering trying CBD, I would recommend Dr. Robert Shmerling’s advice about the dos and don’ts in choosing an appropriate product. Until there is high-quality scientific evidence in humans, it is difficult to make a recommendation for the regular use of CBD in chronic pain management. Each has unique properties, potential [https://gitea.sltapp.cn/philomenamunro Global Health Farms CBD Gummies] benefits, and side effects. The following article covers CBD and THC, their effects, their legal status, and their safety considerations. CBD and THC are known as cannabinoids. They have very similar [https://www.buzznet.com/?s=chemical%20structures chemical structures] and activate the same receptors within the body’s endocannabinoid system. Despite their similarity in structure, CBD and THC exert entirely different effects. The most significant difference between these two compounds is that THC is psychoactive. This means it can affect how the brain works. THC activates the body’s "reward" pathways to produce the "high" that’s associated with marijuana use. It does this by activating a cannabinoid receptor called CB1. When THC activates this type of receptor, things like movement, body temperature, and brain function are affected. In contrast, CBD is not considered psychoactive. It does not cause euphoria or a "high," even at large doses.

Much of the research on CBD and THC is preclinical, meaning it’s been studied in animal models. Large-scale human trials are necessary to determine if CBD or THC can help treat most of these conditions. Though it does act on cannabinoid receptors, CBD mainly affects other receptors, including serotonin receptors. It may help counteract some of the negative effects of THC, including serious side effects like psychosis. CBD has many purported therapeutic properties. Pain. Overall, there isn’t enough [https://www.answers.com/search?q=evidence evidence] that CBD by itself helps with pain. Some research has shown that high doses can improve neuropathy (nerve pain), though more data is necessary. Anxiety. So far, the results are mixed. Some studies show that CBD can improve symptoms of social anxiety disorder and post-traumatic stress disorder (PTSD), while others show no effect. Seizures. Epidiolex, a purified CBD oil, is approved by the Food and Drug Administration (FDA) for some hard-to-treat types of childhood epilepsy.

Brain protection. Animal studies have shown that CBD may have benefits for those who have Alzheimer’s disease or Parkinson’s disease, but human clinical trials are necessary to verify these effects. Effects on animals may or may not translate into benefits for humans. THC is the psychoactive ingredient from the marijuana plant. The possession and use of marijuana and THC-containing products are not legal in all U.S. THC is effective for increasing appetite and for nausea and vomiting related to cancer chemotherapy. Pain. Prescription THC products have been shown to improve neuropathy and quality of life. Spasticity (muscle tightness). Sativex (nabiximols), a mouth spray that combines THC and CBD, is approved for spasticity in Europe. PTSD. A very small trial found that Cesamet reduced nightmares in 10 military personnel with PTSD. Irritable bowel syndrome (IBS). Smoking THC was found to improve symptoms of IBS, but benefits may be only short-lived. People who used THC long-term were more likely to ultimately require surgery for IBS.

Notably, most of the research into THC’s effects has used standardized THC prescription medications. Other THC-containing products available at dispensaries may or may not be beneficial for any of these conditions. More research is undoubtedly necessary. Understanding how CBD and THC are regulated in the United States is a bit tricky. Basically, it comes down to the source of the product, whether it comes from the marijuana plant or hemp plant. In 2018, the U.S. CBD that contains less than 0.3% THC. Plants that contain this low concentration of THC are known as hemp. Historically, hemp has been used to make things like fiber and paper. Notably, marijuana (which comes from a cannabis plant with more than 0.3% THC) remains a Schedule 1 controlled substance. This means that the FDA considers it to have a high risk of abuse and no accepted medical use. Accordingly, CBD and THC that come from a marijuana plant are also Schedule 1 controlled substances.

In my previous article I covered why Light Metro is the best technology of the next major stage of Auckland’s train network. Here I present a couple of lines that could be the basis for a future network. The lines are designed to form a mesh an enable transfers (especially in the CBD) but are of course just ideas. I am estimating costs as $300m/km for elevated sections and $1b/km for underground sections. Hence I have used elevated line wherever possible. As outlined in my previous article Light Metro is Automated, Grade Separated with Short Trains and High Frequencies. It is well suited to Auckland where requirements exceed Light Rail but a full metro would be overkill. The key advantages of Light Metro over street running light rail is it’s high capacity, frequency and [https://git.numa.jku.at/shonabourke438 Global Health Farms] higher speed. Attempting to push Light Rail beyond it’s natural sweet-spot result in a grade-separated system that costs as much as Light Metro but is worse and often costs more to run.

The below table shows the capacity of a Light Metro line (in each direction). For Auckland the stations outside the CBD could be serviced by buses to further increase coverage area. Trains could start at short length and frequency increased as high as possible before longer trains should be used. If the system is run with 4-car trains then each has the capacity over double one of the major Auckland motorways such as the Western or Southern. This line would upgrade the Northern Busway on the North Shore, run under the CBD and connect to the Airport in the South. The line would be grade separated above the road as much as possible since this is cheaper than under-grounding. It would be underground though the central city however. Total length would be around 36km of which around 5.5 would be underground. This would start at the exiting Albany bus centre and follow the Northern busway to Akoranga station.

It would then go along the shore until roughly opposite Sulphur point where it would either go in a tunnel or bridge over the Harbour to Wynyard Quarter. The Northern Busway should be kept South of Akoranga Station for use by buses from Takapuna, Northcote and Brikenhead. This would give the system more capacity and is easier than those people transferring from a bus to a train for such a short ride. Travel time from Albany to the Te Waihorotiu Station (Aotea) should hopefully be around 25 minutes. Once over the Harbour the line should head underground and have a series of stops in the Central City. The Te Waihorotiu CRL station is apparently already future-proofed with space for a North/South line. The station will effectively be the centre of the Auckland System. There should also be a surface Light Rail line nearby on Queen Street. The University station would be quite deep and probably be a an elevator-only station.

South of Grafton Valley the line would go under the domain before going through Newmarket. The line could either be above or below ground though Newmarket but will be above ground once it [https://discover.hubpages.com/search?query=reach%20Manukau reach Manukau] Road. Update: Feedback has convinced me the line should have a stop under Park Road near the Hospital and another at the bottom of Carton Gore Rd. South of Newmarket the line will travel above Manukau Rd and continue South through Onehunga and Mangere Bridge. The Southern Section would have roughly 16 stations and take over 18km and would take around 30 minutes to cover from the Airport to Te Waihorotiu/Aoetea Station. Previous proposals have followed the motorway but I’ve switched this to following roads inside the suburb of Mangere Bridge and giving the suburb 3 stations with the Millar Rd one having good connectivity to Favona. The Mangere Town Centre station would be a branch off the Bader Drive station.

It could be run as a shuttle. This line is intended to fill the gaps to the North of the existing Western Rail Line and use the Motorway corridor. Closer to town it will go above Great South Rd and Karangahape Rd. It will then do an above-ground spiral around the city to improve coverage and transfers. Total length would be around 20km and all above ground. This would run from Westgate to Karangahape Road mainly along the North Western Motorway and Great North Road (GNR). It would be roughly 16km long and 100% overhead.
